Check AI answers against evidence

Before using an answer, check each claim: does the source support it, which conditions are missing, and who needs to confirm?

In this guide

Four steps for one claim

  1. Record the exact claim to check. Split the answer into individual assertions. Check dates, numbers, responsible people and approval authority separately.
  2. Quote the supporting passage. Record the source name, location and version when available. A link existing does not mean its content supports the claim.
  3. Keep unknowns visible. Look for conditions such as “provisional”, “subject to availability” or scope limits. Do not turn a request into a confirmed fact.
  4. Rewrite or ask the right person. If evidence is insufficient, record what needs confirmation and whom to ask. Preserve approval steps before acting.

Several models giving the same answer does not replace checking it against the source.

SYNTHETIC EXAMPLE / NOT A CUSTOMER PROJECT

Knowing the owner does not confirm the date.

Read the two sources from the Atlas demo below. Source B clarifies the roles, but the launch date remains provisional; there is no evidence that the release has been approved.

Source A · Request

“For Atlas, prepare a launch checklist for 24 September. Maya can review.”

Source B · Team note

“Noah owns Atlas. Maya approves release work. The launch date is still provisional.”
Claim to check
“Atlas will definitely launch on 24 September.”
Evidence / supporting passage
Source A only requests a launch checklist for 24 September. Source B says the launch date is still provisional.
Missing conditions
The date is not confirmed. Knowing Noah is the owner and Maya approves release work does not mean the date has been approved.
Confirmation needed / revised wording
Ask Noah to confirm the date; keep Maya’s approval step. Rewrite: “Source A requests a checklist for 24 September; the launch date still needs confirmation.”

Source A requests a checklist for 24 September. Source B says Noah is the owner, Maya approves release work and the date is provisional. Neither source says Maya has approved it.

[ ] Conditions and unknowns have been preserved.
[ ] The person to ask and the appropriate approval step are recorded.
[ ] Data-processing permissions have been checked for the intended environment.

The user checks the answer against the source. This sheet does not automatically verify an answer.

Before sending or using

Use only necessary data and check processing permissions in your chosen environment. A responsible person should review information before it is shared externally. For consequential decisions, use your organization’s appropriate approval process and specialists.

This is a source-checking guide, not an AI tool that verifies itself. The 23 September 2026 version adds the Atlas example and four reusable fields.
